| Outcome | Annually, direct health care costs for a patient with chronic disease average $6,032, approximately 5 times that of a person without a chronic disease. These costs are primarily derived from more frequent hospitalizations and emergency room visits and greater prescription drug use. Nearly 60 percent of all emergency room visits are associated with people with chronic conditions, at a cost of $8.3 billion in 2017. It’s estimated that nearly 30 percent of such visits could be prevented or treated in a lower-cost setting if better managed. The financial toll of these preventable visits is estimated at $2.5 billion |
